The `-include "file"` option implicitly adds a leading #include
to the main source; however, there's no explicit #include, which
left -frewrite-includes emitting an unmatched #endif at the end
of the included text.
This corrects a bug in 4a16b51f.
The #if now has a conditional expression, so a user can add
`-D__CLANG_REWRITTEN_SYSTEM_INCLUDES` to include the system headers
instead of using the expanded content, or
`-D__CLANG_REWRITTEN_INCLUDES` to include all headers.
Also added the filename to the comments it emits, to help identify where
included text ends, making it easier to identify and remove the content of
individual headers.
Checking that the created output matches something is nice, but
this should also check whether the output makes sense.

We've decided to make the core rewriter class and PP rewriters mandatory.
They're only a few hundred lines of code in total and not worth supporting as a
distinct build configuration, especially since doing so disables key compiler
features.
This reverts commit r213150.
Revert "clang/test: Introduce the feature "rewriter" for --enable-clang-rewriter."
This reverts commit r213148.
Revert "Move clang/test/Frontend/rewrite-*.c to clang/test/Frontend/Rewriter/"
This reverts commit r213146.
