712a351c42 Make the selection of type declarations in Sema::getTypeName
deterministic when faced with an ambiguity. This eliminates the
annoying test/SemaCXX/using-directive.cpp failure.

fe3d7d0880 Make parsing a semantic analysis a little more robust following Sema
failures that involve malformed types, e.g., "typename X::foo" where
"foo" isn't a type, or "std::vector<void>" that doens't instantiate
properly.

Similarly, be a bit smarter in our handling of ambiguities that occur
in Sema::getTypeName, to eliminate duplicate error messages about
ambiguous name lookup.

This eliminates two XFAILs in test/SemaCXX, one of which was crying
out to us, trying to tell us that we were producing repeated error
messages.

700792c4e4 Improvements and fixes for name lookup with using directives, from Piotr Rak!
Also, put Objective-C protocols into their own identifier
namespace. Otherwise, we find protocols when we don't want to in C++
(but not in C).

4489e9465f When looking for a tag name via unqualified name lookup, only look in
scopes where the name would be considered a redeclaration if we know
that we're declaring or defining that tag.
