We've stopped doing it in libc++ for a while now because these names
would end up rotting as we move things around and copy/paste stuff.
This cleans up all the existing files so as to stop the spreading
as people copy-paste headers around.
to reflect the new license. These used slightly different spellings that
defeated my regular expressions.
We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

Use the libc++abi visibility macros instead of pragmas or using
visibility attributes directly. Clean up redundant attributes on
definitions (where the declarations already have visibility attributes
applied, from either libc++ or libc++abi headers).
Introduce _LIBCXXABI_WEAK as a drive-by cleanup, which matches the
semantics of _LIBCPP_WEAK.
No functional change. Tested by building on Linux before and after this
change and verifying that the list of exported symbols is identical.

We've been having issues with using libcxxabi and libunwind for baremetal
targets because fprintf is dependent on io functions, this patch disables calls
to fprintf when building for baremetal in release mode.

caused by testing the value of something that was undefined. This
should prevent that in the future.

The NDK doesn't have access to `android/set_abort_message.h`, so use
an extern declaration instead for API 21. For older releases, just use
`__assert2`, which will report to logcat and/or the tombstone for some
older releases.

In Android, stderr only goes to the console, and as such will only ever
be seen by adb shell users. Since very few developers will ever actually
see that, also send the abort message to logcat and the tombstone.
