`IMAGE_REL_ARM64_REL64/IMAGE_REL_AMD64_REL64` do not exist and `.quad a - .` is
currently not representable.
For instrumentation, `.quad a - .` is useful representing a cross-section
reference in a metadata section, to allow ELF medium/large code models. The COFF
limitation makes such generic instrumentations inconvenient. I plan to make a
PGO/coverage metadata section field relative in D104556.

These might occur in seemingly generic assembly. Previously when
targeting COFF, they were silently ignored, which certainly won't
give the right result. Instead clearly error out, to make it clear
that the assembly needs to be adjusted for this target.
Also change a preexisting report_fatal_error into a proper error
message, pointing out the offending source instruction. This isn't
strictly an internal error, as it can be triggered by user input.

Now that we've moved to C++14, we no longer need the llvm::make_unique
implementation from STLExtras.h. This patch is a mechanical replacement
of (hopefully) all the llvm::make_unique instances across the monorepo.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

someone else's reference" trick.

Fixes the same ownership issue for COFF that r315245 did for MachO:
WinCOFFObjectWriter takes ownership of its MCWinCOFFObjectTargetWriter, so we
want to pass this through to the constructor via a unique_ptr, rather than a
raw ptr.
