Removed redundant ops from the standard dialect that were moved to the
`arith` or `math` dialects.
Renamed all instances of operations in the codebase and in tests.

This commit introduced a cyclic dependency:
Memref dialect depends on Standard because it used ConstantIndexOp.
Std depends on the MemRef dialect in its EDSC/Intrinsics.h
Working on a fix.

Create the memref dialect and move several dialect-specific ops without
dependencies to other ops from std dialect to this dialect.

The prior diff that introduced `addAffineIfOpDomain` missed appending
constraints from the ifOp domain. This revision fixes this problem.

This diff attempts to resolve the TODO in `getOpIndexSet` (formerly
known as `getInstIndexSet`), which states "Add support to handle IfInsts
surronding `op`".
Major changes in this diff:
1. Overload `getIndexSet`. The overloaded version considers both
`AffineForOp` and `AffineIfOp`.
2. The `getInstIndexSet` is updated accordingly: its name is changed to
`getOpIndexSet` and its implementation is based on a new API `getIVs`
instead of `getLoopIVs`.
3. Add `addAffineIfOpDomain` to `FlatAffineConstraints`, which extracts
new constraints from the integer set of `AffineIfOp` and merges it to
the current constraint system.
4. Update how a `Value` is determined as dim or symbol for
`ValuePositionMap` in `buildDimAndSymbolPositionMaps`.

All ops of the SCF dialect now use the `scf.` prefix instead of `loop.`. This
is a part of dialect renaming.

Summary: The current syntax for AffineMapAttr and IntegerSetAttr conflict with function types, making it currently impossible to round-trip function types(and e.g. FuncOp) in the IR. This revision changes the syntax for the attributes by wrapping them in a keyword. AffineMapAttr is wrapped with `affine_map<>` and IntegerSetAttr is wrapped with `affine_set<>`.

This simplifies defining expected-* directives when there are multiple that apply to the next or previous line. @below applies the directive to the next non-designator line, i.e. the next line that does not contain an expected-* designator. @above applies to the previous non designator line.

The entire compiler now looks at structural properties of the function (e.g.
does it have one block, does it contain an if/for stmt, etc) so the only thing
holding up this difference is round tripping through the parser/printer syntax.
Removing this shrinks the compile by ~140LOC.
This is step 31/n towards merging instructions and statements. The last step
is updating the docs, which I will do as a separate patch in order to split it
from this mostly mechanical patch.

printing the entry block in a CFG function's argument line. Since I'm touching
all of the testcases anyway, change the argument list from printing as
"%arg : type" to "%arg: type" which is more consistent with bb arguments.
In addition to being more consistent, this is a much nicer look for cfg functions.

access function equality constraints; one more step closer to get slicing
based fusion working in the general case of affine_apply's involving mod's/div's.
