[libc++][modules] Fix missing and incorrect includes (#108850)
This patch adds a large number of missing includes in the libc++ headers
and the test suite. Those were found as part of the effort to move
towards a mostly monolithic top-level std module.

[libc++] Clean up includes of <__assert> (#80091)
Originally, we used __libcpp_verbose_abort to handle assertion failures.
That function was declared from all public headers. Since we don't use
that mechanism anymore, we don't need to declare __libcpp_verbose_abort
from all public headers, and we can clean up a lot of unnecessary
includes.

This patch also moves the definition of the various assertion categories
to the <__assert> header, since we now rely on regular IWYU for these
assertion macros.

[libc++] Format the code base (#74334)
This patch runs clang-format on all of libcxx/include and libcxx/src, in
accordance with the RFC discussed at [1]. Follow-up patches will format
the benchmarks, the test suite and remaining parts of the code. I'm
splitting this one into its own patch so the diff is a bit easier to
review.

This patch was generated with:

[libc++][NFC] Add a few clang-format annotations (#74352)
This is in preparation for clang-formatting the whole code base. These
annotations are required either to avoid clang-format bugs or because
the manually formatted code is significantly more readable than the
clang-formatted alternative. All in all, it seems like very few
annotations are required, which means that clang-format is doing a very
good job in most cases.

[libc++] Rename _LIBCPP_INLINE_VISIBILITY to _LIBCPP_HIDE_FROM_ABI (#74095)
In preparation for running clang-format on the whole code base, we are
also removing mentions of the legacy _LIBCPP_INLINE_VISIBILITY macro in
favor of the newer _LIBCPP_HIDE_FROM_ABI.

We're still leaving the definition of _LIBCPP_INLINE_VISIBILITY to avoid
creating needless breakage in case some older patches are checked-in
with mentions of the old macro. After we branch for LLVM 18, we can do
another pass to clean up remaining uses of the macro that might have
gotten introduced by mistake (if any) and remove the macro itself at the
same time. This is just a minor convenience to smooth out the transition
as much as possible.

4d20cfcf4e [libc++] Simplify single-iterator __bit_iterator algorithm specializations
The single-iterator algorithms have an implementation for `false` and `true`, which are almost identical. Instead of writing two functions, this refactors the code to take the value searched for as a template parameter. This avoids a lot of code duplication and makes it easier to reason about the algorithm and their difference.

56ae3568ea [NFC][libc++][bitset] Refactors constructors.
Based on the review comments in D153201 this combines the string and
c-string constructors. The common constructor is using a string_view:
- it allows propagating the _Traits, which are required to be used for
  comparison.
- it avoids allocating
- libc++ supports it in C++03

385cc25a53 [libc++] Ensure that all public C++ headers include <__assert>
This patch changes the requirement for getting the declaration of the
assertion handler from including <__assert> to including any public
C++ header of the library. Note that C compatibility headers are
excluded because we don't implement all the C headers ourselves --
some of them are taken straight from the C library, like assert.h.

It also adds a generated test to check it. Furthermore, this new
generated test is designed in a way that will make it possible to
replace almost all the existing test-generation scripts with this
system in upcoming patches.

e39095a32e [libc++] Define _LIBCPP_HAS_NO_PRAGMA_SYSTEM_HEADER whenever we enable warnings in the test suite
This should make CI consistent on all the compilers we support. Most of
this patch is working around various warnings emitted by GCC in our code
base, which are now being shown when we compile the tests.

After this patch, the whole test suite should be warning free on all
compilers we support and test, except for a few warnings on GCC that
we silence explicitly until we figure out the proper fix for them.

bd6e6846e7 [libc++] Add the version header to all headers.
Some headers which require the version header depend on other headers to
provide it. Include the version header in all top-level headers to make
sure a header cleanup can't remove the version header.

Note this doesn't add the version header to the c headers.

eb8650a757 [runtimes][NFC] Remove filenames at the top of the license notice
We've stopped doing it in libc++ for a while now because these names
would end up rotting as we move things around and copy/paste stuff.
This cleans up all the existing files so as to stop the spreading
as people copy-paste headers around.

781c476ce0 [libc++] ADL-proof vector<bool> by adding _VSTD:: qualification on calls.
This affects only vectors with weird/malicious allocators,
the same corner case covered in D91708, but for `vector<bool>` this time.

Also ADL-proof <__tree>, which affects only sets and maps with weird/malicious
allocators where the ADL trap is in the *fancy pointer type*.

Also drive-by _VSTD:: qualification in the guts of std::bind,
std::packaged_task, std::condition_variable.

d586f92c94 [libc++] Consistently replace std:: qualification with _VSTD:: or nothing. NFCI.
I used a lot of `git grep` to find places where `std::` was being used
outside of comments and assert-messages. There were three outcomes:

- Qualified function calls, e.g. `std::move` becomes `_VSTD::move`.
    This is the most common case.

- Typenames that don't need qualification, e.g. `std::allocator` becomes `allocator`.
    Leaving these as `_VSTD::allocator` would also be fine, but I decided
    that removing the qualification is more consistent with existing practice.

- Names that specifically need un-versioned `std::` qualification,
    or that I wasn't sure about. For example, I didn't touch any code in
    <atomic>, <math.h>, <new>, or any ext/ or experimental/ headers;
    and I didn't touch any instances of `std::type_info`.

In some deduction guides, we were accidentally using `class Alloc = typename std::allocator<T>`,
despite `std::allocator<T>`'s type-ness not being template-dependent.
Because `std::allocator` is a qualified name, this did parse as we intended;
but what we meant was simply `class Alloc = allocator<T>`.

57b08b0944 Update more file headers across all of the LLVM projects in the monorepo
to reflect the new license. These used slightly different spellings that
defeated my regular expressions.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

63ea958521 [libcxx] Call __count_bool_true for bitset count
This patch aims to help clang with better information so it can inline
__bit_reference count function usage for both std::biset. Current clang
inliner can not infer that the passed typed will be used only to select
the optimized variant, it evaluates the type argument and type check as
a load plus compare (although later optimization phases correctly
optimized this out).

It is mainly to help llvm inliner to generate better code for std::bitset
count for aarch64. It helps on both runtime and code size, since if inline
decides that _VSTD::count should not be inlined the vectorization will
create both aligned and unaligned variants (which add both code size and
runtime costs)

a016efb1dc [Libc++] Use #pragma push_macro/pop_macro to better handle min/max on Windows
Summary:
This patch improves how libc++ handles min/max macros within the headers. Previously libc++ would undef them and emit a warning.
This patch changes libc++ to use `#pragma push_macro`  to save the macro before undefining it, and `#pragma pop_macro` to restore the macros and the end of the header.
