Summary:
Some recent support made usage of `__nvvm_reflect` more consistent. We
should expose it as a builtin rather than forcing users to externally
define the function.
This patch deduces `noundef` attributes for return values.
IIUC, a function returns `noundef` values iff all of its return values
are guaranteed not to be `undef` or `poison`.
Definition of `noundef` from LangRef:

This attribute applies to parameters and return values. If the value representation contains any
undefined or poison bits, the behavior is undefined. Note that this does not refer to padding
introduced by the type’s storage representation.

The motivation of this patch is to reduce the number of `freeze` insts
and enable more optimizations.
Right now when we see -O# we add the corresponding 'default<O#>' into
the list of passes to run when translating legacy -pass-name. This has
the side effect of not using the default AA pipeline.
Instead, treat -O# as -passes='default<O#>', but don't allow any other
-passes or -pass-name. I think we can keep `opt -O#` as shorthand for
`opt -passes='default<O#>` but disallow anything more than just -O#.
Tests need to be updated to not use `opt -O# -pass-name`.

There are only two used in the IR optimization pipeline.
Port these and add them to the default pipeline.

I added -mtriple to some tests since under the new PM, the passes are
only available when the TargetMachine is specified.

Summary:
Previously there were three ways to inform the NVVMReflect pass whether
you wanted to flush denormals to zero:
* An LLVM command-line option
* Parameters to the NVVMReflect constructor
* Metadata on the module itself.
This change removes the first two, leaving only the third.
The motivation for this change, aside from simplifying things, is that
we want LLVM to be aware of whether it's operating in FTZ mode, so other
passes can use this information. Ideally we'd have a target-generic
piece of metadata on the module. This change moves us in that
direction.

Summary:
CUDA 7.0's libdevice uses slightly different IR to call __nvvm_reflect
and that triggers an assertion in nvvm_reflect optimization pass. This
change allows nvvm_reflect pass to deal with both old and new ways to
pass an argument to __nvvm_reflect.

Similar to gep (r230786) and load (r230794) changes.
Similar migration script can be used to update test cases, which
successfully migrated all of LLVM and Polly, but about 4 test cases
needed manually changes in Clang.
(this script will read the contents of stdin and massage it into stdout
- wrap it in the 'apply.sh' script shown in previous commits + xargs to
apply it over a large set of test cases)

Alias with unnamed_addr were in a strange state. It is stored in GlobalValue,
the language reference talks about "unnamed_addr aliases" but the verifier
was rejecting them.
It seems natural to allow unnamed_addr in aliases:
* It is a property of how it is accessed, not of the data itself.
* It is perfectly possible to write code that depends on the address
of an alias.
This patch then makes unname_addr legal for aliases. One side effect is that
the syntax changes for a corner case: In globals, unnamed_addr is now printed
before the address space.
