Refactor code revolving source objects such that most logics will be
reused.
The main change is to expose a single `CreateSource(addr, target)` that
can return either a normal or an assembly source object, and call
`ShouldDisplayAssemblySource()` only from this function instead of
multiple places across the code.
Other functions can use `source.IsAssemblySource()` in order to check
which type the source is.
Protect the various SetBreakpoint functions with the API mutex. This
fixes a race condition between the breakpoint being created and the DAP
label getting added. This was causing `TestDAP_breakpointEvents.py` to
be flaky.
Fixes#131242.
Convert Breakpoint & Watchpoints structs to classes to provide proper
access control. This is in preparation for adopting SBMutex to protect
the underlying SBBreakpoint and SBWatchpoint.
Refactoring breakpoints to not use the `g_dap` reference.
Instead, when a breakpoint is constructed it will be passed a DAP
reference that it should use for its lifetime.
This is part of a larger refactor to remove the global `g_dap` variable
to allow us to create multiple DAP instances.

This commit cleans up the includes in the `lldb-dap` subfolder. The main
motivation was that I got annoyed by `clangd` always complaining about
unused includes while working on lldb-dap.
This adds a layer between `SounceBreakpoint`/`FunctionBreakpoint` and
`BreakpointBase` to have better separation and encapsulation so we are
not directly operating on `SBBreakpoint`.
I basically moved the `SBBreakpoint` and the methods that requires it
from `BreakpointBase` to `Breakpoint`. This allows adding support for
data watchpoint easier by sharing the logic inside `BreakpointBase`.
